Student - Faculty Agreement PDF Link *
You MUST meet with your faculty advisor, review the Student-Faculty agreement, and provide a copy of the agreement signed by your advisor and all student facilitators who have agreed to teach next semester. If you have not finalized your facilitator team for next semester, have everyone who has agreed to teach as of yet sign the agreement. You can provide a Google Drive link or other cloud link here.

POC Facilitator *
If your STIC will have multiple student facilitators, please indicate here who the main point of contact for the class will be. Please provide their name and contact method (email, phone number, etc.)
Please provide three 1-hour time slots that you and any co-facilitators who will be teaching are available for. At least one facilitator must be present for every class. 

CMSC STICs are generally held on Fridays, for classroom availability reasons. If you are electing to hold your class on another day, please state your reason why here. (E.g. Friday 2-2:50PM, Friday 11-11:50AM, Friday 3-3:50PM).
