Hi All,
This form serves as a way for me and the rest of EC to tackle membership problems as soon as they arise instead of waiting until open forum at the end of the semester.  Your feedback will help us to make improvements in the chapter as soon as an issue arises.  That being said, this form should be used in a CONSTRUCTIVE manner- not as an attack on any member of this chapter.  Erin and I are the only ones who have access the responses and we will keep all responses COMPLETELY confidential.  If you have any questions please feel free to reach out to me.
